Heart Health Support at The Memorial Hospital

What is Cardiac Rehabilitation?

Cardiac rehabilitation provides comprehensive care to patients that have recently suffered a heart attack, have ongoing heart problems or have recently had heart surgery. The medically designed and supervised program combines patient education, physical activity and support to improve patient health and reduce the risk of future health problems.

The cardiac rehabilitation team at The Memorial Hospital is made up of primary care physicians, cardiologists, cardiac rehabilitation technicians, and physical and occupational therapists. This team will develop a personalized program for your care.

Many insurance companies cover cardiac rehabilitation programs. Check with your health care provider or cardiologist to see if you have a qualifying condition for cardiac rehab.

A cardiac rehab program generally lasts 2-3 months. During that time the team at TMH will help create a specialized program to help you transition into a normal life routine including returning to work and participating in physical activities.

A prescription for cardiac rehab will be provided by your physician. Your doctor can fax a copy of the order to the scheduling department at (970) 826-3159.
